Treatment Protocols · Bay Area Orthobiologics
You've already been told what surgery looks like. We're here to show you what's possible without it. Below you'll find our full range of regenerative protocols — each one matched to the complexity of your condition and the demands of your life.
The Right Biologic. The Right Dose. The Right Moment.
Not every condition responds to the same biologic. Grade II osteoarthritis is not the same as Grade IV. A rotator cuff tear is not the same as knee cartilage loss. Our clinical process begins with understanding exactly where your body is — then deploying the specific combination of biologics, concentration, and timing that gives your tissue the best possible environment to regenerate. This is the science behind why our outcomes are different.
Four treatment protocols. Each matched to the grade and complexity of your condition. All include a comprehensive consultation, imaging review, ultrasound-guided injection, and Osteopathic Manipulation at follow-up visits. Your biology is unique. Your protocol should be too.
Mild to moderate osteoarthritis (Grade I–II) · Tendon injuries · Early-stage joint degeneration
Moderate to advanced arthritis · Nerve entrapments (carpal tunnel) · Conditions requiring anti-degenerative protection
Grade III osteoarthritis · Significant soft tissue damage · Conditions requiring maximum cellular regeneration
Grade IV osteoarthritis · Large soft tissue tears (rotator cuff, complex joint) · Advanced conditions previously considered surgical-only · Older patients requiring high cell counts
Every treatment protocol at Bay Area Orthobiologics includes Osteopathic Manipulation at your follow-up visits. This isn't an add-on. It's a core part of the clinical protocol — and it's one of the things that makes outcomes here different from a standard regenerative medicine injection clinic.
Osteopathic Manipulation (OMT) is a hands-on treatment technique performed by osteopathic physicians to restore proper movement, alignment, and function to the musculoskeletal system. It uses carefully applied pressure, stretching, and movement of the muscles and joints to release restrictions, improve circulation, and reduce tension in the tissues surrounding an injured area.
In the context of regenerative medicine, that matters more than most people realize. When you receive a PRP or stem cell injection, your body immediately begins mobilizing healing cells to the treatment site. But if the surrounding tissue is restricted — if the joint is poorly aligned, if the muscles are guarding, if circulation is compromised — those healing cells can't get where they need to go efficiently.
As a triple board-certified osteopathic physician, Dr. Fischer doesn't just have the ability to perform OMT — he has the clinical training to understand exactly how structural mechanics interact with the body's regenerative response. That integration is rare in regenerative medicine, and it's built into every protocol here by design.
Your follow-up visits aren't just check-ins. They're active treatment sessions where Dr. Fischer evaluates how your body is responding, addresses any mechanical restrictions that could be limiting your healing, and adjusts your protocol if needed. Most regenerative clinics don't offer this. At Bay Area Orthobiologics, it's standard.
Restricted joints and tight surrounding tissue limit blood flow to the treatment site. OMT opens those pathways — improving delivery of the healing cells and growth factors your body needs to rebuild.
Poor alignment and muscle guarding create mechanical stress on a healing joint. By addressing those restrictions directly, OMT allows the biologic treatment to work in a better environment — less interference, better results.
Regenerative medicine is about giving your body the right tools. OMT ensures your body can actually use them. The combination of precision biologics and osteopathic structural work produces outcomes that neither approach achieves alone.
Osteopathic Manipulation is included at both follow-up visits in every treatment protocol. It is performed by Dr. Fischer personally — not delegated — as part of a fully integrated approach to regenerative care that addresses your injury at every level: biological, structural, and mechanical.
Your initial treatment gives your body the biological tools it needs to begin healing. The Fischer Protocol is what protects those results over time — a clinically designed, personalized ongoing care program prescribed by Dr. Fischer based on your specific condition, your biologic response, and the life you're determined to keep living. This isn't maintenance. This is precision optimization of your body's regenerative potential, year after year.
The Fischer Protocol Advanced is exclusively available to patients who have undergone a BMAC or Adipose Cellular Therapy containing procedure at Bay Area Orthobiologics.
Osteoarthritis is a progressive condition — your body breaks down cartilage faster than it can rebuild it. A single annual treatment helps. But two precision PRP treatments per year, timed every six months, systematically stays ahead of that breakdown cycle in a way that a single booster simply cannot. Think of it like fertilizing a lawn twice a season instead of once — you're not just maintaining what's there, you're continuously building on it. Patients on the Fischer Protocol Advanced don't just feel better after their initial stem cell treatment. They stay better, season after season, because we never let degeneration get a head start. They ski harder. They play more golf. They run further. They age on their own terms — because they have a clinical protocol working for them year round.
